About the Role

We are seeking a highly skilled Pharmacist to join our team as a Home Infusion Therapist. As a key member of our Pharmacy team, you will be responsible for providing pharmaceutical care to patients in their homes, working closely with physicians, nurses, and other healthcare professionals to ensure the safe and effective use of medications.

Responsibilities
  • Evaluate and monitor drug therapy for home care patients, collaborating with healthcare professionals to optimize treatment plans.
  • Adhere to organization policies, state and federal laws, and regulations governing pharmacy practice.
  • Coordinate new referrals and admissions, facilitating seamless transitions for patients.
  • Review, verify, and document patient orders, ensuring accuracy and compliance.
  • Assist healthcare professionals with planning and implementing patient care, setting therapeutic objectives, and monitoring drug therapy outcomes.
  • Conduct assessments, collect and interpret data, and provide consultation on drug-related issues.
  • Maintain thorough documentation of clinical activities and communication with patients and healthcare professionals.
  • Process and check medications, supplies, and equipment prior to dispensing.
Requirements
  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
  • Registered Pharmacist License in state(s) of practice.
  • Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy.
  • Experience with compounding infused drugs.
  • Proven record of evaluating and monitoring drug therapies for home care patients.
  • Good organizational and time-management skills.
  • Participation in professional societies and organizations is desirable.
  • Excellent verbal and written communication skills.
  • Flexible schedule with the ability to work on-call evenings and weekends.
