Job Summary

We are seeking a skilled Registered Nurse to join our team at Olydia Home Care Inc in Brooklyn Park. As a key member of our healthcare team, you will be responsible for creating and managing personalized care plans for patients, providing interventions to maintain or improve their health status, and coordinating medical appointments.

Responsibilities
  • Perform focused nursing assessments to determine appropriate interventions for patients.
  • Provide interventions for patients with episodes of potentially life-threatening instability.
  • Provide nursing visits and direct care to patients in their homes or assisted living facilities, as directed by client care plans.
  • Set up medications for patients according to the medication management program, as indicated.
  • Coordinate medical appointments for patients, including follow-up with new orders or changes after hospital visits, with direction from the nursing case manager as needed.
  • Accept and implement prescriber orders with direction as needed.
  • Respond to phone calls from other providers related to patients, with RN direction as needed.
  • Document, as indicated, in the clinical record.
  • Communicate with families, resident representatives, case managers, and other healthcare providers regarding patients as needed.
  • Be aware of and use good safety practices, including body mechanics, falls prevention, reporting of defective equipment or hazards, wiping spills or wet floors immediately, using Standard Precautions and appropriate infection control measures, knowledge of emergency procedures, and reporting incidents/accidents to the Clinical Nurse Supervisor or Director on the day of occurrence.
  • Participate in care conferences with other members of the healthcare team.
  • Maintain absolute confidentiality of all information pertaining to patients, families, and employees.
  • Promote a safe and comfortable environment for patients.
  • Demonstrate excellent written and verbal communication skills.
Qualifications
  • Currently licensed Registered Nurse (RN) in the state of employment.
  • Previous experience as a Registered Nurse is preferred.
  • Excellent interpersonal and client care skills.
  • Valid driver's license.
  • First aid/CPR certification required.
