We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a key member of our care team, you will play a vital role in providing high-quality patient care and support.
Key Responsibilities- Evaluate and develop individualized treatment plans for patients with speech, language, and swallowing disorders.
- Conduct thorough assessments and provide evidence-based interventions to improve patient outcomes.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and professional development to stay current with best practices and industry developments.
- Master's degree in Speech Language Pathology or related field.
- Current certification in Speech Language Pathology (CCC-SLP) or equivalent.
- Minimum of six months experience in home health or related setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
