About the Role:- As a Physical Therapist, you will be responsible for assessing and treating clients in their homes, providing individualized care plans to promote their health and independence.
- You will work closely with our multidisciplinary team to develop and implement care plans, and provide education and training to clients and their families.
- Our Physical Therapists are passionate about delivering exceptional patient care and making a positive impact on our clients' lives.
- Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
- Provide manual therapeutic exercises, gait training, balance, and other interventions to promote clients' physical function and mobility.
- Collaborate with our nursing team to develop and implement comprehensive care plans that address clients' medical and physical needs.
- Provide education and training to clients and their families on rehabilitative care and activities necessary to promote their health and independence.
- A current Pennsylvania Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
