Job Summary:
In collaboration with physicians and other healthcare professionals, the Nurse Practitioner will provide holistic, compassionate care to patients and families, and coordinate care management activities on behalf of PruittHealth Corporation, Inc.
Key Responsibilities:
- Provide primary and specialty care to patients in various settings, including hospitals, clinics, and long-term care facilities.
- Conduct thorough assessments, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless care delivery.
- Collaborate with physicians, staff RNs, and other healthcare professionals to ensure comprehensive patient care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
- Master's degree in Nursing with a focus on Nurse Practitioner education.
- Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
- Experience in geriatric care and patient care coordination.
- Excellent communication and interpersonal skills.
