CLIENT REQUIRES assistance with physical activities, transportation to healthcare appointments, fitness sessions, and leisurely walks in the community.
Position Overview:
As a vital member of the healthcare team, you will deliver essential home health aide services within the patient's residence, guided by a registered nurse or qualified therapist.
Key Responsibilities:
- Exhibit effective communication skills, capable of reading, writing, and verbally conveying clinical information to patients, family members, and healthcare professionals.
- Monitor, report, and document patient conditions and the care provided.
- Accurately measure and record vital signs including temperature, pulse, and respiration.
- Adhere to standard precautions and infection control protocols.
- Timely report and document any changes in patient health status to the nursing supervisor.
- Ensure a safe, clean, and healthy environment for both patients and colleagues.
- Recognize emergencies and implement appropriate emergency response measures.
- Demonstrate an understanding of the comprehensive needs of patients while respecting their privacy and confidentiality.
- Utilize safe techniques for transferring, ambulating, positioning, and assisting with range of motion exercises while providing personal care.
- Complete necessary documentation promptly and accurately.
- Deliver quality patient care in accordance with the Aide Service Plan and contribute to its updates.
- Fulfill required in-service training hours as per agency guidelines.
- Seek assistance when necessary and comply with agency policies and procedures.
- Engage in interdisciplinary evaluations of patient care and contribute to the development or modification of care plans.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Take on additional responsibilities as assigned.
