Job Summary

We are seeking a highly motivated and experienced Case Manager to join our team. The successful candidate will be responsible for managing a caseload of participants, administering program policies and procedures, and providing evidence-based rehabilitation services.

Key Responsibilities
  • Manage a caseload of participants, including conducting risk/need assessments, creating individual treatment plans, and documenting progress.
  • Provide evidence-based Cognitive Behavioral Therapy (CBT) programs and other life skills programs to participants groups.
  • Conduct group and individual meetings utilizing motivational interviewing techniques.
  • Develop and implement rehabilitative plans with participants.
  • Testify in court regarding participants' compliance with court orders.
  • Screen participants at jail for entry into the program.
  • Provide face-to-face services to participants.
  • Develop treatment plans with participants.
  • Maintain progress notes in participant files.
  • Keep referring officers apprised of participant's progress.
  • Develop an exit plan to include discharge and aftercare information.
Requirements
  • Associate's degree from a granting institution accredited by the Western Association of Schools and Colleges (WASC) or equivalent and a minimum of two (2) years of experience working with the criminal justice population.
  • High school diploma or its equivalent and a minimum of four (4) years' experience working in a similar position with the criminal justice population.
  • CBT Facilitator certification is desirable.
  • Basic knowledge of case management, referral services, and interventions in both group and individual modalities.
  • Basic knowledge of the criminal justice system.
  • Good presentation, public speaking, and facilitation skills.
  • Good organizational skills and attention to detail.
  • Effective communication skills with internal and external contacts at all levels.
  • Ability to handle multiple priorities simultaneously.
  • Problem-solving ability.
  • Good interpersonal skills.
  • Basic computer skills.
  • Bilingual (English/Spanish) skills may be required.
Compensation

The successful candidate will be offered a competitive hourly rate of USD $25.00 per hour.
