Physical Therapist

We are seeking a skilled Physical Therapist to join our team at East Bay Post-Acute.
Job Summary:The successful candidate will be responsible for developing and implementing effective treatment plans for patients, working closely with our nursing department and other healthcare professionals to ensure seamless care.
Key Responsibilities:- Develop and implement individualized treatment plans for patients, in collaboration with referring physicians.
- Treat patients according to established treatment plans, providing hands-on care and education to promote optimal recovery.
- Assist the nursing department in training Restorative Aides, ensuring they have the necessary skills to provide high-quality care.
- Supervise Physical Therapy Assistants in direct patient care and related activities, adhering to state practice acts and facility policies.
- Communicate effectively with supervisors and other healthcare team members regarding patient progress, challenges, and plans.
- Participate in patient care conferences, utilization review meetings, and rehabilitation conferences as needed.
- Engage in in-service training programs for other staff members, promoting a culture of continuous learning and growth.
- Record and maintain accurate documentation of patient evaluations, treatment notes, progress updates, and recertification requirements, in accordance with PT Board state practice acts and governmental regulations.
- Collaborate with patients, families, and nursing staff to develop and implement discharge plans, ensuring a smooth transition to post-therapy care.
- Recommend and facilitate the ordering of necessary durable medical equipment to enhance patient mobility and independence.
- Report any equipment issues to ensure timely maintenance and optimal functionality.
- Exhibit excellent interpersonal and communication skills, fostering positive relationships with patients, families, staff, and professional colleagues.
A valid Physical Therapist license is required for this position.
