  • Regularly gather and assess patient information to ensure comprehensive care.
  • Monitor patient conditions and responses to treatments, suggesting modifications to care plans as necessary.
  • Collaborate with healthcare team members through case discussions and consultations to coordinate patient care.
  • Accurately document all aspects of patient care in compliance with established policies.
  • Uphold patient confidentiality and implement safety protocols consistently.
  • Educate patients and their families on essential care practices.
  • Provide emotional support to patients, their families, and caregivers.
QUALIFICATIONS:

A minimum of one year of clinical experience is required, ideally in hospice, home health, oncology, acute care, or public health settings. Proficiency in English is mandatory, with bilingual skills preferred. Strong time management and communication abilities are essential.

CERTIFICATIONS/LICENSES:

Possession of a valid Florida LPN license and a driver's license is required.

PHYSICAL REQUIREMENTS:

Must be capable of lifting up to twenty-five pounds, with good visual and auditory perception. Ability to perform effectively under varying levels of stress is necessary.
