Home Health Occupational Therapist

About the Role:
We are seeking a skilled Occupational Therapist to join our team at Granite Peak Home Health. As an Occupational Therapist, you will play a vital role in helping our patients achieve their maximum potential and independence.
Key Responsibilities:
- Assess and Evaluate Patients: Conduct thorough assessments and evaluations of patients to identify their strengths, weaknesses, and goals.
- Develop and Implement Treatment Plans: Create and implement personalized treatment plans to help patients achieve their goals and improve their functional abilities.
- Provide Skilled Therapy Services: Provide skilled occupational therapy services to patients, including exercises, activities, and interventions to promote independence and functional ability.
- Collaborate with Healthcare Team: Work collaboratively with the healthcare team, including physicians, nurses, and other therapists, to ensure comprehensive care and coordination of services.
- Document Patient Progress: Accurately and thoroughly document patient progress, including treatment plans, progress notes, and discharge summaries.
Requirements:
- Graduate of an Occupational Therapy Curriculum: Must be a graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
- Current Licensure: Must be currently licensed in the state(s) in which practicing, unless licensure does not apply.
- Experience: Two (2) years' experience, preferred.
