Job Summary

We are seeking a compassionate and skilled Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide direct patient care under the direction of a Registered Nurse and according to the Aide Plan of Care (POC)
  • Assist patients with self-administered medications, including opening bottle caps, reading labels, and observing medication intake
  • Take accurate temperature, pulse, and blood pressure measurements and report abnormal results to the supervisor
  • Help patients maintain good personal hygiene through bathing, grooming, skin care, and other activities
  • Assist with feeding patients, including communicating basic nutrition principles and observing food and fluid intake
  • Provide necessary skills to safely assist patients with mobility, exercises, positioning, transfers, and ambulation
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care
Requirements
  • High school diploma or equivalent
  • Completion of a Certified Nursing Assistant (CNA) program within the last 24 months
  • Valid driver's license, auto insurance, and reliable transportation
  • Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare-certified agency
  • At least one year of experience in the last 24 months as a Home Health Aide or CNA in a hospital, nursing home, or home health/hospice agency
