Position Overview:
This Internal Medicine Advanced Practice Provider will become an integral member of our collaborative healthcare team. You will work alongside physicians, fellow Advanced Practice Providers (APPs), nursing staff, and care coordinators to deliver comprehensive care.
Role in Patient Care:
As a provider in internal medicine, you will foster enduring relationships with adult patients, guiding them on their path to wellness. Your role emphasizes preventive care and the management of chronic conditions, ensuring that you are a primary resource for your patients' overall health.
Chronic Conditions Managed:
You will oversee a variety of chronic health issues, including but not limited to:
- Diabetes
- Hypertension
- Congestive Heart Failure
- Chronic Kidney Disease
- COPD/Asthma
- Depression/Anxiety
- Sleep Apnea
- GERD/IBS
- Stroke/TIA
- Anemia
In addition, you will provide preventive care to enhance patient well-being.
Work Schedule:
This position operates Monday through Friday, with the APP working four 10-hour shifts from Tuesday to Friday. You will also participate in a rotating after-hours call schedule.
Compensation and Benefits:
This is a full-time salaried position with comprehensive benefits, including generous funding for continuing medical education to support your professional development as an APP.
Key Responsibilities:
- Conduct thorough patient assessments to gather complete clinical information.
- Order necessary tests and interpret results to guide patient care.
- Develop and implement care plans based on patient needs.
- Ensure timely and accurate documentation of clinical activities.
- Provide education to patients and their families to enhance understanding and engagement in their care.
Collaboration and Communication:
You will coordinate care with other healthcare providers, ensuring effective communication of clinical information to support patient outcomes.
Research and Professional Development:
Engagement in research initiatives and evidence-based practice is encouraged to promote quality care and continuous improvement in clinical standards.
Qualifications:
- Current state licensure as an Advanced Practice Registered Nurse (APRN).
- Certification as a Nurse Practitioner.
- Eligibility for State Controlled Substance License and Federal DEA Certificate.
- BLS Certification for Healthcare Providers.
- Additional certifications as required.
Preferred Qualifications:
- A Master's degree or higher in a relevant field.
- Certification from a recognized nursing credentialing organization.
- Relevant clinical experience as defined by leadership.
