We are seeking a skilled Physical Therapist to join our team at Transitions Home Health Care. As a PRN Outpatient Physical Therapist, you will have the opportunity to work with patients in their place of residence, providing comprehensive physical therapy treatment and regimens to develop and restore function.
Responsibilities- Evaluate patients and develop individualized plans of care in partnership with patients, representatives, and caregivers.
- Provide skilled therapy services per physician orders, patient plans of care, clinic policies, and professional standards.
- Communicate effectively with patients, families, caregivers, and healthcare team members to ensure seamless care coordination.
- Document all care and services provided according to professional standards and clinic policy and procedures.
- Participate in quality management and maintain a culture of safety and quality.
- Licensed as a Physical Therapist in the State of Colorado.
- Graduate of a Physical Therapist curriculum approved by the Commission on Accreditation in Physical Therapy.
- Two years' experience as a Physical Therapist and previous experience working with the geriatric population preferred.
- Liability Insurance and current Medicare Credentialing a plus.
