Job Summary

The Patient Advocate role at Catholic Health Services is a vital position that requires strong communication and interpersonal skills. As an advocate for the patient, you will assist patients and their family members with their needs, ensuring a supportive environment that prioritizes patient care.

Key Responsibilities
  • Collaborate with the Director of Patient Experience to coordinate empathetic and timely relations between patients and their families.
  • Support and promote patients' rights as they navigate the healthcare system.
  • Enhance the process of communication between the patient/family and the healthcare staff.
  • Interpret information regarding patients' and families' healthcare needs, including patient education, discharge instructions, procedures, emergency encounters, and assessments.
  • Translate basic written information as needed and within department guidelines.
  • Arrange appointments, transportation, and facilitate medication interpreter services when needed.
  • Identify available community-based resources and actively manage appropriate referrals, access, engagement, follow-up, and coordination of services.
  • Track and follow up with prospective patients to meet a wide range of patient needs.

Requirements
  • Physically able to stand for long periods and navigate self throughout the hospital environment.
  • Ensure Compliance with all CHS & SFH standards, including the corporate compliance assurance program, HIPAA, and IT Security Policy.
  • Demonstrate Professional demeanor in verbal and non-verbal communication, sensitivity, compassion, and appropriate problem-solving skills.
