Key Responsibilities:

Schedule independent home visits to screen and treat patients
Continuously reevaluate and document patient progress
Collaborate with an interdisciplinary team to provide comprehensive care
Utilize exceptional communication skills to ensure effective patient care

Requirements:

Bachelor of Science Degree in Occupational Therapy from an accredited institute of learning
Registered and licensed as an OT in NYS
Minimum of one-year experience in providing therapy treatments in a healthcare setting; Home Care experience highly preferred
Ability to provide own transportation and maintain a valid driver's license
