Key Responsibilities:
  • Conduct thorough assessments of patients to determine their home care needs
  • Develop and implement personalized care plans to meet the unique needs of each patient
  • Provide direct patient care, including administering medications and treatments as prescribed by physicians
  • Counsel patients and their families on meeting their nursing and related needs
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  • Participate in on-call duties and ensure that arrangements for equipment and other necessary items and services are available
Requirements:
  • Graduate of an accredited school of nursing with a current licensure in the state
  • One to two years of recent acute care experience in an institutional setting, with home health care experience preferred
  • CPR certification and a valid driver's license with a reliable vehicle
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills
