About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a key member of our care team, you will work closely with patients, families, and healthcare professionals to provide high-quality speech and language services in the comfort of their own homes.

Responsibilities
  • Evaluate and treat patients with speech and language disorders, including those related to stroke, traumatic brain injury, and neurodegenerative diseases.
  • Develop and implement individualized treatment plans to improve communication skills and quality of life.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between settings.
  • Provide education and training to patients and families on communication strategies and techniques.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A) from ASHA.
  • Minimum of six months of experience as a speech therapist or speech-language pathologist.
  • Home health experience preferred.
  • Current and unrestricted license to practice in the state of employment.
  • Current CPR certification.
  • Excellent communication and interpersonal skills.
