Position Overview

Make a meaningful impact in patient care

At Hospice Plus, we view each day as a chance to enhance the personal nature of care and improve the comfort of our patients. We provide personal care, palliative support, and hospice services, ensuring that individuals receive the necessary care with dignity, grace, and love.

Key Responsibilities of the Hospice Aide/ Certified Nursing Assistant:

  • Report to the Administrator, Executive Director, or Patient Care Manager Senior, working under the guidance of the Registered Nurse and/or Clinical Supervisor.
  • Assist patients in maintaining personal hygiene and a clean living environment.
  • Prepare and serve light meals, assisting with feeding as required.
  • Support patients in mobility, including the safe use of walkers or wheelchairs.
  • Perform light housekeeping duties, such as changing linens and emptying trash.
  • Assist with specific treatments as outlined in the aide plan of care, adhering to state regulations.
  • Encourage patient engagement in activities to promote mental alertness.
  • Provide emotional and psychological support to patients and their families.
  • Communicate any changes in the patient's condition to the Registered Nurse and interdisciplinary team.
  • Educate primary caregivers on basic care practices.
  • Implement infection control measures and standard precautions.
  • Maintain accurate documentation of visits and care provided.
  • Participate in staff meetings and quality improvement activities as assigned.
  • Complete documentation promptly as per orientation guidelines.
Candidate Profile

Qualifications for the Hospice Aide/ CNA Role:

  • A compassionate and empathetic approach to patient care.
  • Ability to provide care in various home settings.
  • Demonstrated flexibility and calmness in crisis situations.
  • Valid Certified Nursing Assistant (CNA) license.
  • Preferred experience: six months in hospice, home health, ICU, geriatric, or oncology settings.
  • Additional experience in hospital settings such as Med Surg, PACU, ER, or critical care is advantageous.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
