Nurse Case Manager Role at Nuvance Health

At Nuvance Health, we are seeking a skilled Nurse Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of effective and efficient patient care.

Key Responsibilities:
  • Provide clinically-based case management to support the delivery of effective and efficient patient care.
  • Pace cases from physiological and economic perspectives, ensuring optimal resource utilization.
  • Partner with Social Workers and collaborate with other healthcare team members to identify appropriate utilization of resources and ensure reimbursement.
  • Utilize criteria to confirm medical necessity for admission and continued stay.
  • Develop discharge plans in collaboration with patients, families, and healthcare teams.
Requirements:
  • 3-5 years of experience in a healthcare setting.
  • Associates Degree or equivalent; Bachelors in Nursing preferred.
  • Current RN Licensure required.
Work Environment:

This part-time role is located at Rhinebeck-NDH, with a standard schedule of 18.75 hours per week. The ideal candidate will possess excellent communication and collaboration skills, with a strong focus on patient-centered care.
