Job Summary

We are seeking a skilled Pediatric Physical Therapist to join our team at Angels of Care Pediatric Home Health. As a Physical Therapist, you will play a vital role in implementing treatment programs to assist pediatric patients with physical, neurological, cognitive, and social/emotional disabilities or delays.

Responsibilities
  • Conduct thorough evaluations and interpret assessment results to develop effective treatment plans.
  • Implement skilled interventions to maximize the potential of each individual child, assisting them to develop or regain physical, neurological, cognitive, and/or social/emotional functioning.
  • Participate in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine.
  • Coordinate with referral partners to provide services for children in accordance with the physician order.
  • Assure continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs.
  • Document patient care services and care coordination in an intuitive electronic medical record system.
Requirements
  • State license
  • Current CPR certification
  • A minimum of 1 year of experience preferred
