The Care Manager RN plays a crucial role in providing comprehensive and coordinated care to patients within UNC Health. This position involves utilizing a variety of skills, including risk segmentation, patient assessments, patient-centered care plans, tasks or interventions, care transitions, delegated work, and payer communication.

Key Responsibilities
  • Collaborate with healthcare providers, patients, and their families to ensure that patients receive the highest quality of care and support throughout their healthcare journey.
  • Develop and implement patient-centered care plans that address the unique needs and goals of each patient.
  • Conduct thorough patient assessments to identify potential risks and develop strategies to mitigate them.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Manage delegated work and tasks to ensure timely completion and high-quality outcomes.
  • Develop and maintain relationships with payers to ensure optimal reimbursement and coverage for patients.
Requirements
  • North Carolina RN License – Compact or NC State
  • BLS
  • ASN, agreement to enroll in BSN if not already enrolled or obtained
  • 2-3 years of acute care/patient-population, case specific/bedside care experience
