We are seeking a highly skilled RN Case Manager to join our team at Thinksoft Technologies LLC. As a RN Case Manager, you will play a critical role in ensuring the delivery of high-quality patient care and services.
Key Responsibilities:- Develop and implement individualized care plans for patients with complex medical needs.
- Collaborate with healthcare providers, patients, and families to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments of patients' medical, social, and financial needs to identify areas for improvement.
- Provide education and support to patients and families on disease management, treatment options, and health promotion.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.
- BSN or related Bachelor's degree required.
- 5 years clinical ED, critical care or med/surg experience required.
- Previous case management experience preferred.
- Knowledge of healthcare financial, regulatory, and payer issues preferred.
- Knowledge of state, local, and federal programs strongly preferred.
- Experience in use of Milliman/InterQual criteria.
- Registered Nurse (RN) NYS license required.
- PRI/Screen certification strongly preferred.
- C.C.M. (Certified Case Manager) certification preferred.
- Computer skills essential.
- Competitive salary range: $119,970 - $153,900.
- Sign-on bonus: $5,000.
- Full-time position with a permanent shift of M-F 9A-5P with a weekend rotation (Sat & Sun).
