Position Overview
As a vital member of the Facility Resource Pool, you will provide expert clinical guidance in all facets of patient care within an acute inpatient environment, prioritizing patient and family-centered approaches. You will demonstrate clinical proficiency by employing evidence-based practices throughout the nursing process, which includes assessment, planning, implementation, evaluation, and outcomes. Your role will also involve integrating safety protocols, cost management, and quality enhancement into nursing practices.Compliance with all organizational policies and procedures is essential, including but not limited to: maintaining licensure, completing required training, participating in employee health assessments, adhering to attendance and dress code policies, ensuring patient confidentiality, and following infection control and medication administration protocols. You will also need to be knowledgeable about regulatory agency standards (TJC, DHEC, CMS) and provide care in accordance with the South Carolina Nurse Practice Act.
This position may require flexibility to work across various nursing units within the assigned facility and may involve weekend, holiday, and on-call shifts.Location Information
This position is based at Hillcrest and may involve travel to other medical campuses as necessary.

Key Responsibilities
Conduct comprehensive nursing assessments and collect relevant data using appropriate standards and tools. Document findings accurately and recognize both subtle and significant changes in patient conditions. Collaborate with healthcare team members to establish nursing priorities for individualized care plans, ensuring documentation is thorough and precise.
Care Planning and Outcomes:
Formulate personalized care plans based on the patient's age and specific needs, collaborating with patients, families, physicians, and the interdisciplinary team. Continuously update care plans in response to changes in patient status and communicate these updates effectively to the healthcare team.
Set realistic and measurable goals for patient outcomes prior to discharge, validating these with patients and families to ensure understanding and agreement.
Respect and incorporate the cultural, spiritual, and belief systems of patients and families in care planning.
Plan for discharge to ensure continuity of care and facilitate a smooth transition to the next level of care, including appropriate referrals.
Implementation:
Deliver direct patient care in alignment with established nursing plans, adhering to departmental and organizational policies within the RN scope of practice.
Delegate nursing tasks to team members as appropriate, ensuring clarity in assignments and monitoring task completion.
Continuously monitor the quality of care provided, ensuring patient safety and adherence to established care standards.
Utilize available resources effectively to prioritize patient care, incorporating cost containment strategies into nursing practices.
Employ relevant technology in daily nursing activities and utilize performance indicators to assess quality and effectiveness of care.
Evaluation:
Evaluate the effectiveness of care plans based on patient responses to interventions, documenting progress toward expected outcomes and adjusting plans as necessary. Communicate changes in patient conditions to the healthcare team and discuss outcomes collaboratively.
Patient and Family Education:
Assess and address the educational needs of patients and families, employing clear language and relatable examples. Implement and evaluate teaching plans, encouraging active participation from patients and families.
Ensure that the educational needs of patients and families are communicated effectively within the healthcare continuum.
Professional Accountability:
Reflect on and evaluate your nursing practice against professional standards, state regulations, and organizational policies. Maintain professional licensure and engage in ongoing education to enhance community health outcomes. Document pertinent information accurately to support patient care needs.
Act as an advocate for patients, coordinating care in collaboration with physicians and other healthcare professionals.
Supervisory Responsibilities
This is a non-management position reporting to a supervisor or manager.
Qualifications
Associate's Degree in Nursing is required; a Bachelor of Science in Nursing (BSN) is preferred.
A minimum of two years of experience as a registered nurse is required.
Licensure and Certifications
Must hold a valid Registered Nurse license in South Carolina.
