Responsibilities
  • Provide skilled nursing visits to patients in their homes, utilizing thorough and timely electronic documentation and interdisciplinary communications.
  • Collaborate with RN case managers to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
  • Administer medications and treatments as prescribed, monitoring patients' responses and adjusting care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • LPN professional license in Massachusetts.
  • Minimum 1 year previous nursing experience in acute care or sub-acute care setting.
  • Home care experience preferred.
  • Effective written, verbal, and electronic communication skills.
  • Ability to work with various computer software and knowledge of hand-held patient care computer devices preferred.
