Job Summary

We are seeking a compassionate and motivated Registered Nurse to join our team as a Wound Care Coordinator - RN. As a key member of our nursing center, you will lead and integrate a fully developed Skin Health and Wound management program, collaborating with nursing center management to deliver high-quality patient care.

Key Responsibilities
  • Lead and integrate a comprehensive Skin Health and Wound management program, ensuring adherence to national guidelines and wound care standards.
  • Support central supply as the clinical expert for skin, wound, and incontinent products, ensuring accurate ordering and inventory management.
  • Oversee durable medical equipment related to wound prevention and management, ensuring timely delivery and maintenance.
  • Collaborate with the interprofessional team to develop and implement wound care plans, promoting patient-centered care and skin health.
  • Provide education and training to nursing center staff on wound care best practices, ensuring a culture of excellence in skin health and wound management.
Requirements
  • Must possess a RN license (MPT/DPT with leadership review/approval)
  • Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire
  • Commitment to attend monthly support calls is required
  • Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire
  • Knowledge and experience with clinical charting, incident reporting, and investigation response is required
  • Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team
  • Experience in team building and demonstrating respect for the interprofessional team in wound management is required
  • Must have knowledge of national wound guidelines and wound products
  • Knowledge of nursing home regulations and survey process is required
  • Strong Microsoft Word, Excel, Database Management, PowerPoint skills are required
