Home Health Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Pathwell Health. As a Physical Therapist, you will play a vital role in assessing, planning, and implementing rehabilitative programs to improve mobility, relieve pain, and increase strength for our clients.
Key Responsibilities- Develop and implement individualized conditioning and rehabilitation plans in partnership with clients, caregivers, and physicians.
- Conduct thorough assessments and screenings to evaluate client physical strength and function.
- Educate clients and their representatives on exercise programs and therapeutic procedures to be continued at home.
- Accurately document client care management and services provided.
- Provide direct patient care as prescribed, including evaluation and treatment planning.
- Review physician referrals and patient medical records to determine diagnosis and required physical therapy treatment.
- Record client progress, treatment, and response in patient charts or electronic records.
- Identify and document client goals, anticipated progress, and plans for reevaluation.
- Evaluate treatment effects and adjust plans to achieve maximum benefit.
- Participate in educational activities and clinical studies.
- Graduation from an approved Physical Therapy curriculum.
- Current and valid California Physical Therapist license.
- Recent work experience in home health (preferred).
- Strong clinical assessment, organizational, and communication skills.
- Ability to handle multiple assignments simultaneously.
- Ability to utilize computers and other electronic devices for tasks such as timekeeping and documentation.
- Bilingual (Spanish) preferred.
