Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at ComForCare Home Care. As a LPN, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes.

Key Responsibilities
  • Provide professional nursing care to clients, including personal care services, monitoring and reporting findings, medication administration and monitoring, client teaching, emergency intervention, and nursing services within professional scope of practice.
  • Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in the development, implementation, and evaluation of the Agency Quality Assurance and Performance Improvement (QAPI) program.
  • Collaborate with the clinical supervisor and director of clinical services to ensure high-quality care and adherence to agency policies and procedures.
Requirements
  • Graduate of an accredited school of professional nursing.
  • Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience.
  • CPR certified.
  • Demonstrated written, verbal, and interpersonal communication skills.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Has a good driving record.
