Job SummaryWe are seeking a Licensed Clinical Social Worker to join our interdisciplinary team. As a key member of our rehabilitation team, you will work closely with patients, families, and healthcare professionals to develop and implement individualized treatment plans.
Responsibilities:- Assess and counsel patients with medically complex needs, developing treatment plans to address adjustment to disability, resource linkage, and behavior modification.
- Collaborate with interdisciplinary teams to create integrated treatment plans, incorporating a variety of activities to promote functional goals and durable outcomes.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Master's degree in Social Work and state licensure as an LCSW are required.
- Excellent planning, organization, writing, and time management skills are essential.
- Ability to communicate effectively with diverse stakeholders, including families, professionals, and co-workers.
