Position Overview

The Rehabilitative Care Specialist is responsible for delivering comprehensive rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and standards set forth by Life Care.

Qualifications and Credentials

Must be a graduate from an accredited physical therapy program, holding a BSPT, MSPT, or DPT degree (for those graduating in 2003 or later, an MSPT or DPT is required) Active registration/licensure in the applicable state is mandatory, with a commitment to maintaining a valid license throughout employment. A minimum of one (1) year of experience in a post-acute care or similar environment is preferred. CPR certification is required upon hiring or must be obtained during orientation, and it must remain current throughout employment.

Job Responsibilities

Engage in community health initiatives and projects as appropriate. Proficient in using Microsoft Word, Excel, and email communications. Collaborate with patients, families, and support departments to effectively address patient needs. Demonstrate proper body mechanics consistently. Make autonomous decisions when necessary. Possess knowledge of practices, procedures, and regulations governing operations in a post-acute care setting. Implement and interpret departmental programs, goals, objectives, policies, and procedures. Exhibit proficiency in all competency areas, including patient rights, safety, and sanitation. Maintain professional relationships with colleagues, vendors, and other stakeholders. Uphold confidentiality regarding all proprietary and confidential information. Adhere to company policies, including those related to harassment and compliance. Demonstrate integrity and professionalism by following Life Care's Code of Conduct and completing all mandatory compliance training.

Core Duties

Implement physical therapy treatment plans for patients. Set, evaluate, and adjust realistic, measurable, and functional goals. Supervise and assess the care provided by PTAs, Rehab Aides, and students in PT and PTA programs. Maintain accurate and timely documentation. Utilize therapy software effectively and accurately. Provide excellent customer service and maintain a positive demeanor towards patients. Assist in the safe evacuation of patients when necessary. Exhibit reliable and consistent attendance. Apply reasoning skills and sound judgment in all situations. Communicate effectively and collaborate within an interdisciplinary team. Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving throughout working hours. Demonstrate proficiency in reading, writing, speaking, and understanding the English language.
