Job Summary

We are seeking a highly skilled Integrated Care Professional to join our team at Bayview Hunters Point Foundation for Community Improvement. About the Role

In this role, you will be responsible for assessing clients' needs and developing individualized care plans that address their complex needs. You will work closely with a team of mental health professionals, case managers, and other healthcare providers to ensure seamless coordination of care.

Key Responsibilities

  • Conduct thorough assessments of clients' needs and functioning.
  • Develop and implement individualized care plans that address clients' complex needs.
  • Collaborate with a multidisciplinary team to coordinate care and ensure seamless transitions.
  • Provide education and support to clients and their families to empower them to manage their mental health conditions.

Requirements

To be successful in this role, you will need:

  • A master's degree in social work, psychology, or a related field.
  • A minimum of three years of experience in integrated care settings.
  • Strong assessment and treatment planning skills.

Salary Range

The salary range for this position is $68,000 - $80,000 per year, depending on experience.
