Certified Palliative Care Professional

A compassionate and skilled Registered Nurse is needed to provide expert palliative care to patients in their homes. As a Hospice Services professional, you will work closely with families to develop personalized plans of care, ensuring the highest level of comfort and dignity for each patient.

Our team of experienced healthcare professionals is committed to delivering empathetic and comprehensive services that meet the unique needs of each patient and their loved ones.
Responsibilities- Assess patient needs and develop individualized plans of care, working collaboratively with attending physicians and other healthcare professionals as needed.
- Provide direct nursing care to patients in their homes, focusing on pain management, symptom control, and overall comfort.
- Maintain accurate and up-to-date records of patient care, including assessments, interventions, and outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless coordination of care.
- Promote patient autonomy and independence, supporting them in making informed decisions about their care.
- Foster a culture of empathy and compassion within the hospice team, modeling exceptional service and professionalism in all interactions.
- Demonstrated expertise in palliative care, with a strong focus on patient-centered care and symptom management.
- Current licensure as a Registered Professional Nurse in the State of Georgia.
- At least six months of experience in acute care or home healthcare settings, with preference given to candidates with prior hospice experience.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams.
We offer a competitive salary range of $80,000 - $110,000 per year, depending on experience. Additionally, our benefits package includes:
