MDS Coordinator

We are seeking a highly skilled MDS Coordinator to join our team at Liberty Village of Peoria. As a key member of our healthcare team, you will be responsible for ensuring the accurate and timely completion of Minimum Data Set (MDS) assessments, as well as providing support for OBRA and PPS MDS assessments.
Key Responsibilities- Schedule and assign ARD (assessment reference date) for all MDS assessments
- Complete and review all assigned observations to support OBRA MDS assessments
- Audit and review all ICD 10 coding on admission
- Complete Clinical sequencing for all Medicare A and Managed Care/Like Medicare A
- Complete and monitor MDS transmission process using Matrix Care and SimpleLTC
- Participate in Medicare Meeting per policy
- Facilitate Plan of Care Preview meeting
- Monitor and review Case Mix Index in conjunction with Care Plan Coordinator
- Monitor and review Quality Measures report and 5 star report in conjunction with Care Plan Coordinator
- Occasional lifting and/or moving/pushing and/or pulling up to 50 lbs
- Specific vision abilities required by this job include close vision, depth perception and ability to adjust focus
