Job Summary
We are seeking a skilled Registered Nurse to join our team as an Inpatient RN Case Manager. As a key member of our healthcare team, you will be responsible for coordinating patient care, ensuring seamless transitions, and promoting optimal health outcomes.
Key Responsibilities
- Provide clinical expertise in assessing patient needs, developing care plans, and implementing evidence-based interventions.
- Collaborate with interdisciplinary teams to ensure comprehensive care, including medication management, wound care, and pain management.
- Facilitate communication between patients, families, and healthcare providers to ensure informed decision-making and patient satisfaction.
- Monitor and report on patient progress, identifying areas for improvement and implementing strategies to enhance care quality.
Requirements
- Associate Degree in Nursing or higher required; BSN preferred.
- 3+ years of acute care hospital experience or comparable job-related experience in the last 5 years.
- Registered Nurse licensure in the state of Virginia or a Compact state.
