We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at AllHealth Network. As a Family Nurse Practitioner, you will play a vital role in providing high-quality medical care to our clients in the Acute Treatment Unit.
Job Summary:The Family Nurse Practitioner will be responsible for obtaining medical histories, conducting physical examinations, diagnosing and treating acute and chronic illnesses, and ordering or performing diagnostic tests. You will also be responsible for providing consultation to staff, evaluating and monitoring client medications, and establishing contact with outpatient providers for continuity of care.
Key Responsibilities:- Obtain medical histories and conduct physical examinations of clients in the Acute Treatment Unit.
- Diagnose and treat acute and chronic illnesses, and order or perform diagnostic tests as necessary.
- Provide consultation to staff on medical problems and medications of clients.
- Evaluate and monitor client medications, including side effects and adverse reactions.
- Establish contact with outpatient providers for continuity of care.
- Refer clients to outpatient treatment for ongoing care as appropriate.
- Provide competent and quality medical care to clients.
- Ensure medical services and follow-ups are completed within established timelines.
- Provide interventions and documentation to meet regulatory standards and/or contractual requirements.
- Attend daily clinical rounds and participate with the inter-disciplinary treatment team.
- Collaborate with partner medical facilities, outpatient providers, and insurance companies to provide appropriate continuity of care.
- Maintain current knowledge of psychopharmacology, including medication interactions.
- Complete all required paperwork accurately and within established timeframes.
- Follow all AllHealth Network policies and procedures.
- Master's Degree in Nursing (MSN) or better.
- Licensed as an Advanced Practice Registered Nurse (APN) in the State of Colorado.
- Unrestricted DEA license.
- At least one year post-master's direct clinical experience with adult psychiatric clients in a medical/behavioral health setting.
