Position Details
Shift: 10:45 PM - 7:15 AM
Key Responsibilities
- Establishes rapport with clients, providing support, encouragement, and motivation to foster independence while maintaining professional boundaries.
- Collaborates as a team member to ensure optimal care for every client by conducting rounds, monitoring facilities, facilitating necessary tests, distributing personal care items, enforcing guidelines, and referring clients to appropriate services, all while treating clients with dignity and respect.
- Documents and shares relevant client information through meetings and individual discussions, ensuring confidentiality is upheld.
- Aligns with the Company’s Vision and Mission statements, promoting these values in all interactions.
- Adheres to all organizational policies and procedures, supporting the Company’s vision, mission, and core values.
- Encourages a safety-first culture by modeling positive behavior and ensuring compliance with safety protocols.
- Maintains regular and predictable attendance as a critical job requirement.
- Performs additional duties as assigned.
Education Requirements
- High School Diploma or equivalent is required.
- Associate's degree in human services or a related field, or equivalent experience is preferred.
- Experience working with vulnerable populations, including the homeless, addicted, or mentally ill, is preferred.

- Strong verbal and written communication skills.
- Proficient interviewing and assessment skills with at-risk populations.
- Proficient in Microsoft Office applications.

Additional Requirements
- Valid NYS Driver License in compliance with company driving criteria is preferred.
- Must adhere to CDC guidelines, including wearing a mask while on duty.
- Full vaccination against Covid-19, including booster when eligible, is strongly encouraged.
