We are seeking a skilled and compassionate Weekend Charge Nurse to join our team at Salem Lutheran Homes & Rehab. As a key member of our nursing staff, you will be responsible for providing high-quality patient care, collaborating with physicians and other healthcare professionals, and contributing to a positive and supportive work environment.



Job Responsibilities


  • Conduct comprehensive nursing assessments of residents, develop individualized care plans, and implement interventions to address their physical, emotional, and social needs.
  • Administer medications, treatments, and procedures according to established protocols, and ensure accurate documentation of resident care and treatment.
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and achieve optimal resident outcomes.
  • Provide education and support to residents, family members, and caregivers on health promotion, disease prevention, and management of chronic conditions.
  • Participate in quality improvement initiatives, such as audits and performance improvement projects, to enhance the quality of care and services provided.
  • Assist with the development and implementation of policies, procedures, and standards of care that promote a culture of excellence and resident-centered care.


Requirements


  • Graduate of an accredited school of nursing, with a valid RN license in the state of practice.
  • Minimum of 2 years of experience in a long-term care or acute care setting, with a strong background in gerontological nursing.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Proficiency in electronic medical records (EMR) systems and other technology used in healthcare settings.
  • Commitment to ongoing learning and professional development, with a desire to stay current with best practices in nursing and healthcare.
