Job Summary

We are seeking a highly skilled and compassionate Clinical Case Manager - Behavioral Health to join our team at CVS Health. As a Clinical Case Manager, you will be responsible for providing high-quality care coordination and case management services to members who have experienced a behavioral health crisis event.

Your primary goal will be to facilitate appropriate member behavioral healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services. You will work closely with the family/member identified formal/informal supports involved in the member's care and partner with Utilization Management to ensure timely and safe discharge to the next level of care.

This is a field-based position requiring 25-50% travel within a 45-mile radius from the candidate's residence to conduct face-to-face visits. You will need to reside in the state of Illinois and be willing and able to travel as required.
