Job Summary

We are seeking a skilled and compassionate Case Manager to join our team at Unity Care Group. As a Case Manager, you will provide direct support and intensive case management services to Transitional Aged Youth (TAY) ages 16 to 21 years, helping them meet individualized goals and connect with community resources.

Key Responsibilities
  • Establish and maintain relationships with youth, utilizing a variety of approaches to ensure a high level of engagement and support.
  • Maintain individual contact with each youth at weekly intervals, discussing goals, tracking progress, and connecting them with community resources.
  • Support youth in developing independent living skills, including budgeting, transportation, and employment.
  • Participate in relevant team meetings, such as Child Family Team meetings and emancipation conferences.
  • Ensure compliance with CCL Regulations and maintain accurate and billable case notes per Medi-Cal requirements.
Requirements
  • Bachelor's Degree in a relevant field, such as Marriage, Family and Child Counseling, Child Psychology, or Social Work.
  • Experience working with the TAY population, preferably in a public or private child welfare social services setting.
  • Knowledge of Microsoft Office applications and ability to maintain accurate records and documentation.
  • Excellent interpersonal skills, with the ability to facilitate and link services among a diverse variety of mental health programs.
Preferred Qualifications
  • Master's Degree in a relevant field.
  • Two years' experience working with at-risk youth or adolescent children in a residential group home setting.
  • Bi-Lingual in Spanish and English.
