As a Clinical Team Leader with Enhabit Home Health & Hospice, you'll play a critical role in ensuring the delivery of high-quality patient care in the comfort of our patients' own homes. You'll work closely with our team to coordinate care, review patient information, and provide oversight to ensure that our patients receive the best possible care.

Responsibilities
As a Clinical Team Leader, you'll be responsible for:
- Coordinating home health services to ensure seamless delivery of care
- Reviewing and approving patient information to ensure accuracy and compliance
- Providing oversight and guidance to our clinical team to ensure high-quality patient care
- Collaborating with our team to develop and implement care plans that meet the unique needs of our patients
- Staying up-to-date on regulatory requirements and ensuring compliance with all relevant laws and guidelines
Qualifications
To be successful in this role, you'll need:
- A graduate degree in nursing and a valid state license
- A minimum of two years of nursing experience, with at least one year in home health
- Strong knowledge of Federal, State, and local laws and regulations governing home health care
- Excellent communication and leadership skills
- Ability to work independently and as part of a team
