Key Responsibilities:
- Coordinate psychosocial assessment and discharge planning for patients
- Provide leadership and psychosocial support to patients and their families
- Act as a resource to patients, utilizing knowledge of community and healthcare resources to address identified needs
- Facilitate quality resource utilization, including length of stay and level of care
Requirements:
- Master's degree in social work, counseling, or a related field
- Previous social work experience required
- Maryland Social Work licensure required (LMSW or LCSW-C)
