About the Role

At Prime Healthcare, we are committed to delivering high-quality patient care and exceptional service. As a Social Worker II, you will play a vital role in our multidisciplinary team, providing emotional support, crisis intervention, and counseling to patients and their families.

Responsibilities

• Provide psychosocial assessments and develop treatment plans in collaboration with the healthcare team

• Offer emotional support and crisis intervention to patients and their families

• Coordinate discharge planning and provide referrals for financial assistance and supportive services

• Maintain accurate and timely documentation of patient interactions and progress

Requirements

• Master's degree in Social Work or related field

• Current state license required (LSW or higher)

• Minimum one year hospital or outpatient experience in mental health care

• Ability to work with diverse populations and complex problems
