Your Tailor Made Senior Service is seeking a skilled Registered Nurse to provide high-quality care to seniors in their homes. As a Home Health RN, you will be responsible for delivering comprehensive nursing care as prescribed by the physician, ensuring the patient's safety and well-being.

Key Responsibilities:
  • Conduct thorough nursing assessments and develop, implement, and evaluate plans of care in conjunction with the physician, patient, and caregivers.
  • Ensure quality and safe delivery of home health care services in compliance with all applicable laws, rules, regulations, and policies.
  • Collaborate with the interdisciplinary team to develop the plan of care for the patient.
  • Manage nursing treatments and activities to meet the needs of each individual patient.
  • Evaluate utilization of patient services and patient progress, and review evaluation with the care team, patient, family members, physician, and case manager as needed.
Requirements:
  • Valid RN license in Texas.
  • OASIS/Medicare Certified Visits Experience.
  • Electronic Charting experience - We use Finale Health.
  • Developed computer skills.
  • Minimum of 1 year of recent home health nursing experience.
  • Driver's License and Vehicle with Current Auto Insurance.
  • American Heart Association BLS.
  • Problem solving skills.
  • Excellent Communication Skills.
  • Positive, Professional attitude.
