Job Overview

The Clinical Pharmacist will work independently and collaboratively with physicians and other healthcare providers to ensure optimal medication therapy for patients.

Key Responsibilities
  1. Conduct patient interviews and counseling to optimize medication use.
  2. Review medical records and medication profiles to identify potential medication-related issues.
  3. Collaborate with physicians and other healthcare providers to resolve medication-related problems.
  4. Develop and implement comprehensive medication use management plans for high-risk patients.
  5. Assist with care coordination and disease state education to achieve optimal patient outcomes.
Requirements
  • Education: Doctorate degree from an accredited school of pharmacy.
  • Experience: 5 years of recent relevant experience.
  • Certification/Licensure: PHARMD-Licensed Pharmacist or PHARMR-Registered Pharmacist.
  • Skills/Knowledge: Comprehensive knowledge of pharmacological principles, pharmacotherapy, medical terminology, and disease states.
