Optometrist Opportunity

Wal-Mart Stores, Inc. is seeking an Independent Optometrist to lease space inside our Walmart stores. As an independent lease holder, you will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule. You will also have the opportunity to opt into equipment, instrumentation, administrative, and/or Opt-Tech support services for a fair market value.

Responsibilities
  • Provide eye and vision care services to patients
  • Establish and maintain a patient schedule
  • Set and maintain a fee schedule
  • Opt into equipment, instrumentation, administrative, and/or Opt-Tech support services (if state allows)
Requirements
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school (if applicable)
  • Valid state ophthalmology license (if applicable)
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
