Key Responsibilities
  • Provide comprehensive patient care, including assessment, planning, intervention, and evaluation, to patients undergoing home dialysis therapy.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop strategies to address any identified gaps in care.
  • Administer medications and treatments as prescribed, and monitor patients' responses to therapy.
  • Provide education and training to patients and their families on dialysis therapy, including proper use of equipment and medication management.
  • Participate in quality improvement initiatives to enhance patient outcomes and facility operations.
  • Collaborate with other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.
Requirements
  • Graduate of an accredited School of Nursing.
  • Current appropriate state licensure.
  • Current or successful completion of CPR BLS Certification.
  • Minimum of 12 months experience as a Registered Nurse.
  • Successful completion of 3 months experience as a Registered Nurse in home peritoneal dialysis and/or hemodialysis.
Preferred Qualifications
  • Home dialysis therapy experience.
  • Experience working in a nephrology setting.
