GENERAL OVERVIEW:
The Clinical Pharmacist is accountable for delivering comprehensive, patient-centered pharmaceutical services during their assigned shifts across various sectors of the pharmacy department.
This role operates under the general supervision of the administrative or clinical coordinator of the area, while also providing guidance and oversight to the support staff on duty.
The pharmacist is expected to demonstrate sound clinical judgment and effective management skills when making decisions in the absence of coordinators or senior personnel.
Additionally, they assist coordinators in identifying and resolving issues within their designated practice area.
The pharmacist plays a key role in mentoring pharmacy interns and externs across all pharmacy service sectors.KEY RESPONSIBILITIES:
Oversees the distribution of unit dose medications, intravenous solutions, investigational drugs, and other pharmaceuticals to fulfill patient care requirements.
Assesses and tracks the pharmaceutical needs of patients.
Reviews patient medical records and/or consults with patients, physicians, or nursing staff to gather necessary information for medication profiles. Analyzes drug-related data to identify potential issues with drug therapy and proposes suitable solutions.
Confirms medication orders for accuracy and compliance. Checks prepared medications for correct dosage, administration route, frequency, and duration of treatment.
Provides direct supervision to support staff assigned during their shift.
Participates in the educational training of pharmacy students, residents, fellows, and support personnel as assigned.
Offers drug and poison information to patients, physicians, nurses, and other healthcare professionals.
Conducts regular reviews of nursing units in a timely manner as designated.
Assists in evaluating the performance of support staff in the area.
Continuously assesses and monitors patients' responses to prescribed medications according to established standards of practice. Provides pharmacokinetic dosing consultations for patients as required.
EDUCATION AND EXPERIENCE (Required and Preferred)
A degree from an accredited college of pharmacy is required, along with completion of an internship and/or residency in hospital pharmacy.
Strong verbal and written communication skills are essential for effectively disseminating drug information to all levels of healthcare personnel within the institution.
A comprehensive understanding of disease pathophysiology and pharmacotherapy is necessary, along with the application skills required for effective drug therapy monitoring.
Basic teaching abilities are required for instructing and supervising undergraduate pharmacy students, interns, and pharmacy technician trainees.
Skills for providing both informal and formal instruction on drug therapy and inpatient operational activities are also necessary.
The ability to collaborate with others to enhance patient services is essential.CERTIFICATIONS/LICENSES REQUIRED:
Must hold a valid license or be eligible for licensure in the State of Michigan.
If eligible, licensure must be obtained within 30 days of employment; failure to secure licensure will result in termination of the pharmacist position.
