At Services for the UnderServed, we are seeking a highly skilled and compassionate Senior Case Manager Lead to join our team. This role is responsible for providing supervisory oversight and training to Case Managers and Housing Specialists in the housing assessment and documentation process.
The ideal candidate will have a strong background in human services, with experience working with chronically homeless individuals or families. A bachelor's degree in a human service field is required, with a master's degree preferred.
This position involves:
- Supervising and Training: Provide weekly one-on-one supervision to Case Managers and Housing Specialists, as well as group supervision and training sessions.
- Service Plan Development: Collaborate with Case Managers to develop initial service plans and conduct regular reviews.
- Referral and Intake: Supervise Case Managers in referring consumers for medical, mental health, and supportive social services, as well as assisting with the financial entitlement and medical benefit application process.
- Counseling and Support: Provide individual and group counseling, workshops, and recreational activities to clients, as well as schedule and assign groups and activities to Case Managers as needed.
- Chart Documentation: Ensure that client charts are up-to-date, accurate, complete, and timely.
- Administrative Tasks: Approve timesheets, time off requests, and schedules, as well as provide professional development opportunities for staff.
