About the Role

">

The Hospice and Home Care Triage Registered Nurse is a critical member of our team, responsible for receiving and evaluating patient calls, providing counseling and guidance as needed, and coordinating with healthcare providers to ensure seamless care delivery. This role requires strong problem-solving skills, a compassionate demeanor, and the ability to work independently with minimal supervision.

Key Responsibilities

  • Provides expert-level clinical support to patients and families via phone or in-person visits">
  • Evaluates patient information to determine the need for hospitalization, emergency department visit, or other interventions">
  • Counsels patients and families on disease management, treatment options, and end-of-life care">
  • Coordinates with healthcare providers to ensure timely and effective care delivery">
  • Maintains accurate records of patient interactions, including phone logs and care plans">

Requirements

To be successful in this role, you must have a strong nursing background, excellent communication skills, and the ability to work in a fast-paced environment. A valid Illinois and Missouri RN license is required, as well as experience in hospice care and home health services. Preferred qualifications include a Bachelor's degree in Nursing and certifications in Cardiopulmonary Resuscitation (CPR). We offer a competitive salary range of $90,000 - $120,000 per year, depending on experience, plus benefits and opportunities for professional growth.
