Ardent Health Services is seeking a dedicated Family Medicine Geriatric Physician to provide exceptional care for older adults. Our organization is committed to enhancing the quality of life for our patients through comprehensive healthcare services.
Key Responsibilities:- Manage an established patient panel focusing on Medicare Advantage and Medicare beneficiaries.
- Deliver preventive and primary care tailored to the needs of older adults.
- Conduct Annual Medicare Wellness Visits and Comprehensive Geriatric Assessments.
- Oversee Transition of Care Management visits to ensure continuity of care.
- Engage in HCC re-documentation to maintain accurate patient records.
- Achieve targets set by Ardent Health Services for selected Quality Measures.
- Collaborate effectively within a multi-disciplinary team environment.
- Participate in the Chronic Care Management Program to support patient health.
- Board certification in Internal Medicine or Family Practice is required.
- Added Qualification in Geriatrics is preferred but not mandatory.
- Completion of a Geriatric Medicine Fellowship is desirable but not essential.
- Experience in a Senior Care Center is advantageous.
- Interest in pursuing a Medical Directorship role is a plus.
