Residential Case Manager

Agape of Appleton, Inc. is seeking a dedicated and compassionate Residential Case Manager to lead our Agape 7 CBRF program. As a key member of our team, you will provide case management, program management, staff supervision, and direct care support to individuals with differing abilities.
Key Responsibilities:- Develop and implement Individual Support Plans (ISPs) to ensure individuals receive authorized care and services.
- Provide guidance and support to direct care staff, promoting community integration and maximizing social functioning.
- Advocate for individuals' human and civil rights, fostering their dignity and self-worth.
- Respond to crisis situations and provide leadership to direct care staff.
- Collaborate with staff to provide direct care support and promote a positive work environment.
- Bachelor's degree in a human services-related field, such as social work, sociology, or psychology.
- Three to five years of experience in case management, program management, staff supervision, and caregiving.
- Valid driver's license with an excellent driving record.
- Ability to pass a thorough background check.
- Mental and physical capability to assist supported individuals based on their needs.
- $21.00 to $25.00/hour, plus overtime for hours worked in excess of 40 per week.
